ABSTRACT

Objective: To summarize and compare the main traditional Chinese medicineTCMsyndromes of Delta and Omicron variants of severe acute respiratory syndrome coronavirus 2SARS-CoV-2 carriers to provide references for the syndrome evolution and syndrome differentiation of SARS-CoV-2 infection. Method(s):The TCM medical records of imported and local cases of infection with Delta and Omicron variants of SARS-CoV-2 in Changsha since September 23,2021 to March 27,2022 were collected,including 18 Delta variant cases and 36 Omicron variant cases. Their TCM diagnosis information and TCM pathogenesis were analyzed and compared. Result(s): The common manifestations in Delta variant cases were cough,fever,chest distress/shortness of breath,sore muscles,nausea,dry mouth,dry or sore throat,thick and greasy tongue coating,and rapid and slippery pulse. The predominant pathogenesis was dampness-heat in the upper-energizer and heat stagnation in the lesser Yang combined with dampness. The occurrence of chest distress/shortness of breath,greasy tongue coating,slippery pulse,and the proportion of dampness-heat in the upper-energizer syndrome were higher in Delta variant cases than in Omicron variant cases P<0.05. The common manifestations in Omicron variant cases were itchy and sore throat,nasal congestion,running nose,fever,mild aversion to cold,dry mouth,dizziness,slightly reddish tongue with thin white coating,and rapid or wiry pulse. The predominant pathogenesis was wind-dryness invading defensive exterior,and heat stagnation in the lesser Yang. The occurrence of white-coated tongue and the proportion of wind-dryness invading defensive exterior syndrome were higher in Omicron variant cases than in Delta variant casesP<0.05. Conclusion(s): There are certain differences in TCM syndromes and the corresponding pathogenesis between Delta variant and Omicron variant cases in Changsha,Hunan. The Delta variant of SARS-COV-2 tends to induce dampness-heat syndrome, whereas Omicron variant infection tends to elicit wind-dampness syndrome,which is expected to provide a reference for the pathogenesis evolution of SARS-COV-2 infection.Copyright © 2022, China Academy of Chinese Medical Sciences Institute of Chinese Materia Medica. ABSTRACT

Objective: To analyze the clinical manifestations, diagnostic methods and treatment process of the patients with non-Hodgkin's lymphoma complicated with human coronavirus(HCoV)-HKU1 pneumonia and improve the clinical medical staff's awareness of the disease, and to reduce the occurrence of clinical adverse events. Method(s): The clinical data of a patient with non-Hodgkin's lymphoma complicated with HCoV-HKU1 pneumonia with hot flashes and night sweats, dry cough and dry throat as the main clinical features who were hospitalized in the hospital in January 2021 were analyzed, and the relevant literatures were reviewed and the clinical manifestations and diagnosis of HCoV-HKU1 were analyzed. Result(s): The female patient was admitted to the hospital due to diagnosed non-Hodgkin's lymphoma for more than 2 months. The physical examination results showed Karnofsky score was 90 points;there was no palpable enlargement of systemic superfical lymph nodes;mild tenderness in the right lower abdomen, no rebound tenderness, and slightly thicker breath sounds in both lungs were found, and a few moist rales were heard in both lower lungs. The chest CT results showed diffuse exudative foci in both lungs, and the number of white blood cells in the urine analysis was 158 muL-1;next generation sequencing technique(NGS) was used the detect the bronchoalveolar lavage fluid, and HCoV-HKU1 pneumonia was diagnosed. At admission, the patient had symptoms such as dull pain in the right lower abdomen, nighttime cough, and night sweats;antiviral treatment with oseltamivir was ineffective. After treatment with Compound Sulfamethoxazole Tablets and Lianhua Qingwen Granules, the respiratory symptoms of the patient disappeared. The re-examination chest CT results showed the exudation was absorbed. Conclusion(s): The clinical symptoms of the patients with non-Hodgkin's lymphoma complicated with HCoV-HKU1 pneumonia are non-specific. When the diffuse shadow changes in the lungs are found in clinic, and the new coronavirus nucleic acid test is negative, attention should still be paid to the possibility of other HCoV infections. The NGS can efficiently screen the infectious pathogens, which is beneficial to guide the diagnosis and treatment of pulmonary infectious diseases more accurately.Copyright © 2023 Jilin University Press. ABSTRACT

Objective: To retrospectively analyze the clinical data of 52 patients with coronavirus disease-2019 COVID-19 and explore the clinical efficacy of modified Sanxiaoyin on mild/moderate COVID-19 patients. Method(s): The propensity score matching method was used to collect the clinical data of mild or moderate COVID-19 patients enrolled in the designated hospital of the Second Hospital of Jingzhou from December 2019 to May 2020. A total of 26 eligible patients who were treated with modified Sanxiaoyin were included in the observation group,and the 26 patients treated with conventional method were the regarded as the control. The disappearance of clinical symptoms,disappearance time of main symptoms,efficacy on traditional Chinese medicineTCMsymptoms,hospitalization duration,laboratory test indicators,and CT imaging changes in the two groups were compared. Result(s): The general data in the two groups were insignificantly different and thus they were comparable. After 7 days of treatment,the disappearance rate of fever,cough, fatigue,dry throat,anorexia,poor mental state,and poor sleep quality in the observation group was higher than that in the control groupP<0.05,and the difference in the disappearance rate of expectoration and chest distress was insignificant. For the cases with the disappearance of symptoms,the main symptomsfever, cough,fatigue,dry throat,anorexia,chest distressdisappeared earlier in the observation group than in the control groupP<0.01. After 7 days of treatment,the scores of the TCM symptom scale of both groups decreasedP<0.01,and the decrease of the observation group was larger that of the control groupP<0.01. All patients in the two groups were cured and discharged. The average hospitalization duration in the observation group12.79+/-2.68dwas shorter than that in the control group15.27+/-3.11dP<0.01. The effective rate in the observation group92.31%,24/26was higher than that in the control group76.92%,20/26. After 7 days of treatment,the lymphocyteLYMcount increasedP<0.05,and white blood cellWBCcount and neutrophilNEUTcount decreased insignificantly in the two groups. Moreover,levels of C-reactive protein CRP,erythrocyte sedimentation rateESR,and procalcitoninPCTreduced in the two groups after treatmentP<0.01and the reduction in the observation group was larger than that in the control group P<0.01. Through 7 days of treatment,the total effective rate on pulmonary shadow in the observation group 90.00%,18/20was higher than that in the control group77.27%,17/22P>0.05and the improvement of lung shadow in the observation group was better than that in the control groupP<0.01. Conclusion(s):Modified Sanxiaoyin can significantly alleviate fever,cough,fatigue,anorexia,chest distress,poor sleep quality,and other symptoms of patients with mild or moderate COVID-19,improve biochemical indicators,and promote the recovery of lung function. ABSTRACT

Objective: To analyze the distribution characteristics of syndrome of TCM of the patients with COVID-19 in Kunming, China. Methods: To classify and summarize the TCM syndrome types of COVID-19 patients via Cluster analysis combining with tongue image, chest CT and clinical expertise of 36 Covid-19 patients in Kunming. Results: In the 36 cases of Kunming region confirmed COVID-19 patients, 17 cases had fever (47.2%), 18 cases had cough (50%), 16 cases felt bitter taste in the mouth (44.4%), 18 cases felt dry throat (50.0%), 17 cases had poor appetite (47.2%), 15 cases had nausea (41.7%);12 cases had diarrhea (33.3%), 15 cases had insomnia (41.7%);12 cases had chest tightness (33.3%);3 cases had dyspnea (8.3%);6 cases had nasal congestion and running nose (16.7%);15 cases had fatigue (41.7%);6 cases had headache and body pain (16.7%);5 cases had red tongue (13.9%);18 cases had pale red tongue (50%);8 cases had tongue with red edge and tip (22.2%);3 cases had dark red tongue (8.3%);2 cases had cyanosis (5.6%);3 cases had swollen tongue (8.3%);18 cases had dentate tongue (50%);4 cases had yellow tongue coating (11.1%);5 cases had yellow sticky tongue coating (13.9%);12 cases had white sticky tongue coating (33.3%);6 cases had thin white tongue coating (16.7%);2 cases had no tongue coating (5.6%). The chest CT results showed that: There were five cases without lesions. The lesions were located in the upper lobe of one lung in 13 cases located in the lower lobe of one lung in seven cases, located in the upper middle lobe in three cases, located in the lower lobe in five cases, and in the upper middle and lower lobes of double lung in 13 cases. There were 14 cases of Shaoyang syndrome, 17 cases of wet Resistance Tir-juao Syndrome. According to the time of onset, the disease was followed by Shaoyang Syndrome (1 day), the Wet blocked tri-jiao Syndrome (3 days), epidemic poison retention lung syndrome and syndrome of flaring heat in qifen and yingfen (5 days), and dampness-toxicity lung-stagnation syndrome (6 days). Conclusion: The TCM syndromes of COVID-19 in Kunming are mainly the Wet Resistance Tri-Jiao Syndrome and Shaoyang syndrome, followed by dampness-toxicity lung-stagnation syndrome, epidemic poison retention lung and syndrome of flaring heat in qifen and yingfen.
